Well, here's my theory (and of course I'm right). He's got a major rupture or missing length in his vas defrens (probably from vasectomy complications) and pumped a cum-like liquid into his urethra (perhaps with a bulb syringe) with enough force that it filled up his scrotum like a balloon. He'd have to pinch off his urethra (probably with pressure between his balls) until his cock could get hard enough to close his urethra. The overfilled scrotum itself created enough pressure that when his orgasm began (which allows the contents of the vas defrens to enter the urethra), an abnormal amount of "jizz" gushed out his urethra on each contraction, augmented by his hand's pressure on his scrotum. You'll notice he presses down on his scrotum as its deflating. The more it deflates, the less internal pressure it has to squirt without his help.
